The intraspinal extradural lipomas are so rare lesions that the incidence and therapeutic guideline are not well established. The authors present the case of a 63-year-old woman suffered by back pain for last a few years. MRI showed 7cm sized high signal intensity well maginated lobulating contoured mass in right paraspinal space, on L2 level. This mass was continued with epidual mass compressing dural sac, making so called dumb-bell shaped mass. The right second nerve root was encased by the mass. These very unique MR findings suggested Intraspinal extradural lipoma. In consideration of age and mild symptom, the surgical resection was avoided and careful follow-up was optioned
